Access to continuous physiological monitoring and to the full spectrum of organ support. Ventilator waveform analysis, haemodynamic monitoring by invasive and non-invasive methods, real-time organ dysfunction scoring, and renal replacement therapy data are available only in the intensive care setting. A topic requiring none of these is probably a general medicine or emergency medicine question in disguise.
What outcomes fit a critical care thesis?
Mortality and length of stay are the broad endpoints, but they need large numbers and rarely differentiate well. More useful are secondary endpoints that vary continuously and respond to interventions: ventilator days, vasopressor duration, time to shock reversal, daily organ dysfunction scores, and device-associated infection rates. These are objective, respond within the admission, and produce actionable findings.
What sample size is realistic in intensive care?
A general medical and surgical intensive care unit admitting several hundred patients a year supports two to four hundred participants over eighteen months for most observational designs. Point prevalence surveys and audit studies can be done in a single day or week and need no follow-up at all.
